Q: Where is the health center located?
A: The Canyon Health Center is located on 3300 W Camelback Rd. Phoenix, AZ 85086 inside the Prescott Hall.
Q: What do you do at the health center?
A: The Canyon Health Center is basically run like any doctor's office or clinic. We see patients for a wide variety of health problems and preventative care. Services we provide include: diagnosing and treating illnesses; ordering and interpreting diagnostic tests such as EKGs, X-rays and blood work; prescribing medications; performing sports, school or work physicals; blood pressure screening; referrals to specialists; wellness promotion events; routine pap tests; and many more.
Q: How much does it cost to be seen at the Health Center?
A: No charge for students with the student health insurance or for employees with the Cigna Access standard or premier plans. There is a $20.00 co-pay for those students or staff who have private insurance.
Q: What services does the Canyon Health and Wellness Center charge for?
A: Like most health care providers, the Health Center charges for office visits with a provider and for some other medical services we provide - immunizations, EKGs, urinalysis, rapid strep testing, and some medical supplies and medical equipment.
Q: Do I need an appointment to be seen at the Health Center?
A: Appointments are recommended (especially to see a Nurse Practitioner), but walk-ins are welcome and will be seen on a first come, first served basis. A patient with a scheduled appointment will be seen prior to any walk-ins except in the event of an emergency. Call 602.639.6215 to make an appointment.
Q: Do you only see patients who have the student health insurance plan?
A: No, we can see any GCU student, staff, or faculty member regardless of their insurance. Fees may apply (see above).
Q: I don't have insurance. Can I receive healthcare services at the Canyon Heath and Wellness Center?
A: Absolutely. The Canyon Health and Wellness Center provides services to all GCU students, staff, and faculty regardless of their health insurance status.
Q: Will you bill my insurance company for my visit?
A: No. The Canyon Health and Wellness Center does not bill insurance companies for the services provided. The only exception to this is for patients on the student health insurance plan.
Q: What forms of payment can I use to pay my bill?
A:You can pay your Health Center charges on the day of service by cash, check, or Lopes Card. If you do not pay on the day of your visit, your charges will be transferred to your GCU Account.
Q: Can I bring in my spouse or child to be seen in the Health Center?
A: Unfortunately, we can only see GCU students, faculty, and staff. We are unable to see family members or other visitors as patients. The only exception to this is during flu season when we offer flu shots to the general public.
Q: Can I get my prescriptions filled at the Health Center?
A: We are unable to fill prescriptions written by providers outside of the Health Center. However, we do stock some commonly prescribed antibiotics and other medications that may be purchased directly from us if prescribed by one of our providers.
Q: Can I get blood drawn at the Health Center?
A: We do not draw blood at the Canyon Health and Wellness Center. However, if one of our providers orders blood work; you will be sent to Sonora Quest Laboratory to have the blood drawn. Sonora Quest will bill your insurance for the services they provide.
Q: My child is under 18 years of age. Can he or she be seen at the Health Center?
A: In order for any student under the age of 18 years to be seen at the Canyon Health and Wellness Center, Arizona law requires parental consent for medical treatment to be delivered. Therefore, a Parent Authorization Form providing parental consent for medical care must be filled out by the parent or legal guardian.
Q: How do I get a copy of my medical records?
A: It is a simple matter of filling out the Consent for Medical Release Form. These forms can be picked up in the Canyon Health Center during normal business hours, or downloaded from the forms section of this website. In order to process your request, you must provide your name, address, and phone number. In addition, if you wish for your records to be released to another party (i.e. an outside clinician, attorney, insurance company, etc.) their complete address and phone number is required. You also have the option of requesting your entire medical chart or just specific information from your chart.
Q: How do I get a copy of my medical records from my outside doctor sent here to the Canyon Health and Wellness Center?
A: This is accomplished by filling out the Consent for Medical Release Form. These forms can be picked up in the Canyon Health Center during normal business hours, or downloaded from the forms section of this website. In order to process your request in a timely manner, the following information is required: the name of the hospital or doctor from where the records are being requested, a complete address and/or fax number, and a phone number. In addition, your name, address, and phone number are also required. As a courtesy, this form can be mailed or faxed by our Health Center staff at no cost.
