- Provide documentation of successful completion of pharmacology coursework which shall include pharmacokinetics principles and their clinical application and the prescription of pharmacological agents in the prevention and treatment of illness, and the restoration and maintenance of health. The coursework shall contain a minimum of:
a. Three (3) graduate credit hour pharmacology course offered by an accredited college or university within two years immediately prior to the date of application to the Board; or
b. Forty-five (45) contact hours [a contact hour is fifty (50) to sixty (60) minutes] in a pharmacology course which includes a competency component, offered by an accredited college or university, within two (2) years immediately prior to the date of application to the Board; or
c. Three (3) graduate credit hours pharmacology course, included as part of an advanced practice nursing education program, within five (5) years immediately prior to the date of application to the Board.
- Provide documentation of a minimum of three hundred (300) clock hours preceptorial experience in the prescription of drugs, medicines and therapeutic devices with a qualified preceptor, to be initiated with the pharmacology course and to be completed within one year of the beginning of the course. Preceptorial experience completed as a part of the formal educational program in which the pharmacology course is taught will meet the three hundred (300) clock hour requirement.
- Submit a collaborative practice agreement with a practicing physician who is licensed under the Arkansas Medical Practices Act, §17-95-201 et seq., and who has training within the scope, specialty or expertise of the advanced practice registered nurse. APRNs who will prescribe controlled substances shall seek a collaborative practice with a physician who has an unrestricted DEA registration number. The collaborative practice agreement shall include, but not be limited to:
a. Availability of the collaborating physician(s) for consultation or referral or both;
b. Methods of management of the collaborative practice, which shall include the use of protocols for prescriptive authority;
c. Plans for coverage of the health care needs of a client in the emergency absence of the advanced practice registered nurse or physician;
d. Provision for quality assurance;
e. Authorization for the APRN to prescribe hydrocodone combination products which were reclassified from Schedule III to Schedule II as of October 6, 2014 if expressly authorized by the collaborating
physician; and
f. Signatures of the advanced practice registered nurse and collaborating physician(s), signifying mutual agreement to the terms of the collaborative practice.
- APRNs issued a certificate of prescriptive authority after December 31, 2015 shall obtain a minimum of three (3) hours of prescribing education which includes information on maintaining professional boundaries and the prescribing rules, regulations and laws that apply to APRNs in the state of Arkansas within two (2) years of issuance of the prescriptive authority certificate.

-have a collaboration agreement with a physician unless the advanced practice nurse is authorized to practice in a hospital, hospital affiliate, or ambulatory surgical treatment center.
-obtain a mid-level practitioner controlled substance license. Medication orders shall be reviewed periodically by the collaborating physician.
To obtain a mid-level practitioner controlled substance license, please follow these steps:
1. Supporting Document CCA must be completed and submitted with each application. Your application will not be processed without completion of this form.
2. Complete Parts II through V of application and the supplemental documentation.
3. Submit the appropriate $5 licensure fee. Make check or money order payable to the Department of Financial and Professional Regulation. Fee is not refundable.
4. Return application, supporting documents and fee to the below noted Springfield, Illinois, address.
5. Failure to properly complete the application will delay licensure.
6. If applying for schedule II prescriptive authority, submit an official transcript with school seal affixed to document and
completion of 45 graduate hours in pharmacology.
NOTE:
- A mid-level practitioner controlled substances license will not be issued until your advanced practice nurse license has been issued.
- If the collaborating physician has delegated prescriptive authority to the advanced practice nurse, the written collaborative agreement shall include a statement indicating that the collaborating physician has delegated prescriptive authority for legend drugs and/or Schedule II, III, IV, and V controlled substances. The delegation must be within the physician's scope of practice and within the scope of the advanced practice nurse's training.
- The written collaborating agreement shall be signed by both the physician and the advanced practice nurse and a copy maintained at each location where the advanced practice nurse practices and shall be provided to the Illinois Department of Financial and Professional Regulation upon request. A copy of the advanced practice nurse Illinois and federal controlled substances licenses numbers shall be kept with the agreement.
- If the advanced practice nurse is delegated prescriptive authority of Schedule II controlled substances the following guidelines apply. Specific Schedule II controlled substances by oral dosage or topical or transdermal application may be delegated. This delegation must identify specific Schedule II controlled substance by either brand or generic name and must be attached to the collaborative agreement. Schedule II controlled substances to be delivered by injection or other route of administration may not be delegated. Evidence of completion of at least 45 graduate contact hours in pharmacology must be submitted to obtain Schedule II prescriptive authority. The collaborating physician may only delegate controlled substances that he or she prescribes. Any prescription must be limited to no more than a 30-day supply, with any continuation authorized only after prior approval of the collaborating physician.
- If collaborating with more than one physician, a separate notice of delegation of prescriptive authority shall be submitted when prescriptive authority is delegated.
- If prescriptive authority includes Schedule II, III, IV and/or V controlled substances, the advanced practice nurse will be required to apply for a mid-level practitioner controlled substances license in accordance with the Illinois Controlled Substances Act; however, only one controlled substances license will be issued regardless of the number of collaborating physicians.

(5) If the applicant holds a baccalaureate degree only, submits proof of certification as a nurse practitioner or certified nurse midwife by a national organization recognized by the board and which requires a national certifying examination.
(6) Submit proof of having successfully completed a graduate level pharmacology course consisting of at least two (2) semester hours of academic credit from a college or university accredited by the Commission on Recognition of Postsecondary Accreditation:
(A) within five (5) years of the date of application; or
(B) if the pharmacology course was completed more than five (5) years immediately preceding the date of filing the application, the applicant must submit proof of the following:
(i) Completing at least thirty (30) actual contact hours of continuing education during the two (2) years immediately preceding the date of the application, including a minimum of at least eight (8) actual contact hours of pharmacology, all of which must be approved by a nationally approved sponsor of continuing education for nurses.
(ii) Prescriptive experience in another jurisdiction within the five (5) years immediately preceding the date of the application.
(7) Submit proof of collaboration with a licensed practitioner in the form of a written practice agreement that sets forth the manner in which the advanced practice nurse and licensed practitioner will cooperate, coordinate, and consult with each other in the provision of health care to patients. Practice agreements shall be in writing and shall also set forth provisions for the type of collaboration between the advanced practice nurse and the licensed practitioner and the reasonable and timely review by the licensed practitioner of the prescribing practices of the advanced practice nurse.
(8) Written practice agreements for advanced practice nurses applying for prescriptive authority shall not be valid until prescriptive authority is granted by the board.

(4) The content of the course of study shall be specific to the applicant's nursing specialty.
(5) The instruction provided in the course of study shall include all of the following:
(a) A minimum of thirty-six contact hours of instruction in advanced pharmacology that includes pharmacokinetic principles and clinical application and the use of drugs and therapeutic devices in the prevention of illness and maintenance of health;
(b) Instruction in the fiscal and ethical implications of prescribing drugs and therapeutic devices;
(c) Instruction in the state and federal laws that apply to the authority to prescribe;
(d) Instruction that is specific to schedule II controlled substances, including instruction in all of the following:
(i) Indications for the use of schedule II controlled substances in drug therapies;
(ii) The most recent guidelines for pain management therapies, as established by state and national organizations such as the Ohio pain initiative and the American pain society;
(iii) Fiscal and ethical implications of prescribing schedule II controlled substances;
(iv) State and federal laws that apply to the authority to prescribe schedule II controlled substances;
(v) Prevention of abuse and diversion of schedule II controlled substances, including identification of the risk of abuse and diversion, recognition of abuse and diversion, types of assistance available for prevention of abuse and diversion, and methods of establishing safeguards against abuse and diversion.
-An applicant who practiced or is practicing as a clinical nurse specialist, certified nurse-midwife, or certified nurse practitioner in another jurisdiction or as an employee of the United States government shall include with the application evidence of having completed a two-hour course of instruction approved by the board in the laws of this state that govern drugs and prescriptive authority;

The 30 contact hours may be obtained in a formal academic setting as a discrete offering or as noncredit continuing education offerings and shall include the following course content:
(1) Applicable federal and state laws;
(2) Prescription writing;
(3) Drug selection, dosage, and route;
(4) Drug interactions;
(5) Information resources; and
(6) Clinical application of pharmacology related to specific scope of practice.
-Develop a practice agreement between the nurse practitioner and the patient care team physician as required in 18VAC90-40-90; and
-File a completed application and pay the fees as required in 18VAC90-40-70 of this chapter.
