In order to prescribe and dispense drugs in Arkansas, an APRN must:
- Apply online for Prescriptive Authority- this may done at the time of the application for APRN licensure or at a later date.
- Provide evidence from the national certifying body that differential diagnosis and prescribing practices are recognized as being within the scope of practice for the applicant’s certification category.
- Provide documentation of successful completion of pharmacology coursework which shall include pharmacokinetics principles and their clinical application and the prescription of pharmacological agents in the prevention and treatment of illness, and the restoration and maintenance of health. The coursework shall contain a minimum of:
a. Three (3) graduate credit hour pharmacology course offered by an accredited college or university within two years immediately prior to the date of application to the Board; or
b. Forty-five (45) contact hours [a contact hour is fifty (50) to sixty (60) minutes] in a pharmacology course which includes a competency component, offered by an accredited college or university, within two (2) years immediately prior to the date of application to the Board; or
c. Three (3) graduate credit hours pharmacology course, included as part of an advanced practice nursing education program, within five (5) years immediately prior to the date of application to the Board.
- Provide documentation of a minimum of three hundred (300) clock hours preceptorial experience in the prescription of drugs, medicines and therapeutic devices with a qualified preceptor, to be initiated with the pharmacology course and to be completed within one year of the beginning of the course. Preceptorial experience completed as a part of the formal educational program in which the pharmacology course is taught will meet the three hundred (300) clock hour requirement.
- Submit a collaborative practice agreement with a practicing physician who is licensed under the Arkansas Medical Practices Act, §17-95-201 et seq., and who has training within the scope, specialty or expertise of the advanced practice registered nurse. APRNs who will prescribe controlled substances shall seek a collaborative practice with a physician who has an unrestricted DEA registration number. The collaborative practice agreement shall include, but not be limited to:
a. Availability of the collaborating physician(s) for consultation or referral or both;
b. Methods of management of the collaborative practice, which shall include the use of protocols for prescriptive authority;
c. Plans for coverage of the health care needs of a client in the emergency absence of the advanced practice registered nurse or physician;
d. Provision for quality assurance;
e. Authorization for the APRN to prescribe hydrocodone combination products which were reclassified from Schedule III to Schedule II as of October 6, 2014 if expressly authorized by the collaborating
physician; and
f. Signatures of the advanced practice registered nurse and collaborating physician(s), signifying mutual agreement to the terms of the collaborative practice.
- APRNs issued a certificate of prescriptive authority after December 31, 2015 shall obtain a minimum of three (3) hours of prescribing education which includes information on maintaining professional boundaries and the prescribing rules, regulations and laws that apply to APRNs in the state of Arkansas within two (2) years of issuance of the prescriptive authority certificate.

California

The California Board of Registered Nursing issues a furnishing number to a nurse practitioner that allows him or her to "order" or furnish drugs and devices to patients using approved standardized procedures. A nurse practitioner with a furnishing number may obtain a Drug Enforcement Administration (DEA) registration number if they want to order controlled substances as needed for patient care. For applicants who completed a California-Based Nurse Practitioner Education Program or a Non-California Based Nurse Practitioner Education Program and the Advanced Pharmacology Course was completed within the last 5 years of the date of your application, please provide the following:
• Nurse Practitioner Furnishing Number Application form completed by the applicant.
• Advanced Pharmacology Course Verification form completed by the director of the Nurse Practitioner program.

California Business and Professions Code, Section 2836.1 (g) (3) requires nurse practitioners who are certified by the board and hold an active furnishing number, who are authorized through standardized procedures or protocols to furnish Schedule II controlled substances, and who are registered with the United States Drug Enforcement Administration, to complete, as part of their continuing education requirements, a course including Schedule II controlled substances, and the risks of addiction associated with their use, based on the standards developed by the board. This three-hour course is available online through the California Association for Nurse Practitioners.

3. Provide information regarding other healthcare licenses held.
4. Select population focus, if applicable.
5. Attest to holding an active RN license for at least 3 years.
6. Attest to the completion of 750 hours of prescribing experience while engaged in a mutually-structured mentorship with a Colorado-licensed physician or advanced practice nurse with full prescriptive authority with a corresponding focus.

If you practice at more than one business/practice, you need only a single CSR to prescribe at all of the locations. -have a collaboration agreement with a physician unless the advanced practice nurse is authorized to practice in a hospital, hospital affiliate, or ambulatory surgical treatment center.
-obtain a mid-level practitioner controlled substance license. Medication orders shall be reviewed periodically by the collaborating physician.

To obtain a mid-level practitioner controlled substance license, please follow these steps:
1. Supporting Document CCA must be completed and submitted with each application. Your application will not be processed without completion of this form.
2. Complete Parts II through V of application and the supplemental documentation.
3. Submit the appropriate $5 licensure fee. Make check or money order payable to the Department of Financial and Professional Regulation. Fee is not refundable.
4. Return application, supporting documents and fee to the below noted Springfield, Illinois, address.
5. Failure to properly complete the application will delay licensure.
6. If applying for schedule II prescriptive authority, submit an official transcript with school seal affixed to document and
completion of 45 graduate hours in pharmacology.

NOTE:  A mid-level practitioner controlled substances license will not be issued until your advanced practice nurse license has been issued.
 If the collaborating physician has delegated prescriptive authority to the advanced practice nurse, the written collaborative agreement shall include a statement
indicating that the collaborating physician has delegated prescriptive authority for legend drugs and/or Schedule II, III, IV, and V controlled substances. The delegation
must be within the physician's scope of practice and within the scope of the advanced practice nurse's training.
 The written collaborating agreement shall be signed by both the physician and the advanced practice nurse and a copy maintained at each location where the
advanced practice nurse practices and shall be provided to the Illinois Department of Financial and Professional Regulation upon request. A copy of the advanced
practice nurse Illinois and federal controlled substances licenses numbers shall be kept with the agreement.
 If the advanced practice nurse is delegated prescriptive authority of Schedule II controlled substances the following guidelines apply. Specific Schedule II controlled
substances by oral dosage or topical or transdermal application may be delegated. This delegation must identify specific Schedule II controlled substance by either
brand or generic name and must be attached to the collaborative agreement. Schedule II controlled substances to be delivered by injection or other route of
administration may not be delegated. Evidence of completion of at least 45 graduate contact hours in pharmacology must be submitted to obtain Schedule II
prescriptive authority. The collaborating physician may only delegate controlled substances that he or she prescribes. Any prescription must be limited to no more
than a 30-day supply, with any continuation authorized only after prior approval of the collaborating physician.
 If collaborating with more than one physician, a separate notice of delegation of prescriptive authority shall be submitted when prescriptive authority is delegated.
If prescriptive authority includes Schedule II, III, IV and/or V controlled substances, the advanced practice nurse will be required to apply for a mid-level practitioner
controlled substances license in accordance with the Illinois Controlled Substances Act; however, only one controlled substances license will be issued regardless of the
number of collaborating physicians.

(4) Submit proof of a baccalaureate or higher degree in nursing.
(5) If the applicant holds a baccalaureate degree only, submits proof of certification as a nurse practitioner or certified nurse midwife by a national organization recognized by the board and which requires a national certifying examination.
(6) Submit proof of having successfully completed a graduate level pharmacology course consisting of at least two (2) semester hours of academic credit from a college or university accredited by the Commission on Recognition of Postsecondary Accreditation:
(A) within five (5) years of the date of application; or
(B) if the pharmacology course was completed more than five (5) years immediately preceding the date of filing the application, the applicant must submit proof of the following:
(i) Completing at least thirty (30) actual contact hours of continuing education during the two (2) years
immediately preceding the date of the application, including a minimum of at least eight (8) actual contact hours
of pharmacology, all of which must be approved by a nationally approved sponsor of continuing education for nurses.
(ii) Prescriptive experience in another jurisdiction within the five (5) years immediately preceding the date of the application.
(7) Submit proof of collaboration with a licensed practitioner in the form of a written practice agreement that sets forth the manner in which the advanced practice nurse and licensed practitioner will cooperate, coordinate, and consult with each other in the provision of health care to patients. Practice agreements shall be in writing and shall also set forth provisions for the type of collaboration between the advanced practice nurse and the licensed practitioner and the reasonable and timely review by the licensed practitioner of the prescribing practices of the advanced practice nurse.
(8) Written practice agreements for advanced practice nurses applying for prescriptive authority shall not be valid until prescriptive authority is granted by the board.

Iowa

Controlled Substance Authority is a separate application process. It is a multi-step process which requires the APRN to apply to two state agencies and one federal agency. The APRN should review the application requirements and fees associated with applying for a Louisiana CDS license (LBP) and federal DEA registration (DEA) BEFORE submitting an application for CS privileges to LSBN to ensure he/she will be able to complete all steps within 60 days.
STEP 1: APRN submits application forms, attestation, and letter of explanation to LSBN for review.

An application for Advanced Practice Registered Nurse Prescribing Privileges must be submitted to the Board for review. In order to be eligible to apply, the APRN must also submit documentation that the applicant has completed at least two semester credits (or equivalent quarter hours) in advanced pharmacotherapeutics (as part of an advanced practice registered nurse program, or in the past 24 months). Once an APRN license is issued, and the applicant qualified for prescribing privileges, the Board will notify the Board of Pharmacy that the APRN is eligible for prescribing privileges. Then the APRN must apply for prescribing privileges with the Board of Pharmacy. The APRN may not prescribe any medications until the Board of Pharmacy has issued a license to prescribe, the APRN has submitted a copy of that license to the Board of Nursing and the status has been updated to reflect current active prescribing privileges. Finally, if the APRN wishes to prescribe controlled substances he/she must apply with the Board of Pharmacy and the Drug Enforcement Administration (DEA). He/she may not prescribe controlled substances until the Board of Pharmacy has issued a license to prescribe, he/she has received a DEA certificate to prescribe controlled substances, he/she has submitted a copy of that license to the Board of Nursing and his/her status has been updated to reflect current active prescribing privilege. If the APRN is requesting to prescribe schedule II controlled substances they must submit documentary evidence that they have practiced at least 2 years or 2,000 hours as an APRN or a signed protocol with a collaborating physician for the prescribing of schedule II controlled substances. -Be approved by the board, or by an OBN approver as defined in rule 4723-14-01 of the Administrative Code, or offered by an OBN approved provider unit, as defined in rule 4723-14-01 of the Administrative Code that is headquartered in the state of Ohio; and

-Be at minimum two hours in length.

-Applicants must submit documentation of successful completion to the board in the form of an original certificate, issued by the provider of the course of instruction.

(b) Instruction in the fiscal and ethical implications of prescribing drugs and therapeutic devices;

(c) Instruction in the state and federal laws that apply to the authority to prescribe;

(d) Instruction that is specific to schedule II controlled substances, including instruction in all of the following:

(i) Indications for the use of schedule II controlled substances in drug therapies;

(ii) The most recent guidelines for pain management therapies, as established by state and national organizations such as the Ohio pain initiative and the American pain society;

(iii) Fiscal and ethical implications of prescribing schedule II controlled substances;

(iv) State and federal laws that apply to the authority to prescribe schedule II controlled substances;

(v) Prevention of abuse and diversion of schedule II controlled substances, including identification of the risk of abuse and diversion, recognition of abuse and diversion, types of assistance available for prevention of abuse and diversion, and methods of establishing safeguards against abuse and diversion.

-provide at least fifteen hours of education in controlled substances acceptable to the board as part of the twenty hours required for prescriptive authority if the NP, CNM, or CNS has equivalent controlled substance prescribing authority in another state;

-provide at least fifteen hours of education in controlled substances acceptable to the board as part of the forty-five contact hours required for prescriptive authority if the NP, CNM, or CNS initially is applying to prescribe in Schedules II - V controlled substances.

If a CNP has less than 1,040 licensed practice hours within the role of the CNP, prescriptive authority is overseen through a Collaborative Agreement. To meet the requirements for prescriptive authority one must:
-Hold a current, unrestricted license as a nurse practitioner in the Commonwealth of Virginia; and

-Provide evidence of one of the following:

a. Continued professional certification as required for initial licensure as a nurse practitioner; or
b. Satisfactory completion of a graduate level course in pharmacology or pharmacotherapeutics obtained as part of the nurse practitioner education program within the five years prior to submission of the application; or
c. Practice as a nurse practitioner for no less than 1000 hours and 15 continuing education units related to the area of practice for each of the two years immediately prior to submission of the application; or
d. Thirty contact hours of education in pharmacology or pharmacotherapeutics acceptable to the boards taken within five years prior to submission of the application. The 30 contact hours may be obtained in a formal academic setting as a discrete offering or as noncredit continuing education offerings and shall include the following course content:

(1) Applicable federal and state laws;
(2) Prescription writing;
(3) Drug selection, dosage, and route;
(4) Drug interactions;
(5) Information resources; and
(6) Clinical application of pharmacology related to specific scope of practice.

-Develop a practice agreement between the nurse practitioner and the patient care team physician as required in 18VAC90-40-90; and

-File a completed application and pay the fees as required in 18VAC90-40-70 of this chapter.

Washington

Schedule II substances require a collaborative agreement. If the applicant is seeking prescriptive authority, he/she must submit with the initial ARNP application one of the following:
1. Proof of graduation within the last two years from the ARNP program and completion of an advanced pharmacology course,
2. Proof of 30 hours of continuing education in pharmacology completed within the last two years. Evidence must show pharmacology hours earned, OR
3. Proof of completion of 250 hours of advanced clinical practice with prescriptive authority within the last two years and a request for an exemption to the 30 hours of continuing education in pharmacology. (Prescriptive authority in the other state must be equivalent to Washington to include independent practice and ability to prescribe schedule II-V).

West Virginia
